Also alarmingly, in order to keep from reporting her death occurring at the center, staff may have called on the Kanawha County Emergency Ambulance Authority to transport the patient to the hospital as though she was still alive. “She actually died on the gurney before the ambulance left the facility.” According to what we learned, it is sometimes common practice to give the appearance that the resident is still alive, when they have actually passed away inside of the nursing home.
Our sources also stated that there were signs of neglect that appeared to be severe redness on her legs, from the pelvis area down to her knees. Most commonly caused from being urine soaked for over a period of time. It is also claimed that, as this deceased patient was being transported, her bed was stripped of the linens, replaced with clean, and a new resident was being assigned in that room.

They also rated the following facilities as "Much Below Average" (Because Actual Harm and/or Immediate Jeopardy exists) Federal Regulations do not require State inspectors or the nursing home to notify patients and their families that a particular patient's care has been the subject of a survey violation. Even a finding of substandard care or actual harm does not require notification.
